3. TIME OFF WORK
Those who require a medical letter for their employer or insurance forms completed, please notify the office staff. Allow 3 to 4 business days for the completion of such forms/letters.

4. MISCELLANEOUES CHARGES:

• Deposit - $500 for use of Dry Shipper to Transport Sperm/Embryos
• Courier Fees – if applicable, charged to patient
• Copies of Medical Records - $50.00
• Insurance letters $10.00
• Long Distance Charges (whether phone calls or faxes – for calls made outside of IVF treatment)
• returned cheques for any reason $25.00

5. UROLOGY SERVICES
Epididymal Sperm Aspirations or Testicular Biopsies are billed by the urologist directly and payable to him.

6. Non-Funded patients who do not succeed in achieving a “Clinical Pregnancy” (i.e. confirmed by ultrasound five weeks post transfer) by completion of their second trial may qualify for a reduction in fees on trial three.

7.  Replacement of Treatment Package  
$25.00 if a package was sent to you, and you have misplaced it a fee of $25.00 is applicable.

8. OFF-SITE BLOOD TESTS/ULTRASOUNDS
Please be advised that if you choose to exercise the option of having some of your blood tests and ultrasounds done off-site (ie: not at IVF Canada), you are responsible for paying the laboratory who performs the tests at the time the tests are performed.  If you have all of your blood tests and ultrasounds done at IVF Canada, the costs of those tests are included in the fees you pay us; however, that is NOT the case when the testing is done outside of our facility.

9. PORTABLE INCUBATOR AND DRY SHIPPER
Any patients other than IVFC or LIFE PROGRAM patients that are using our portable incubator to transport eggs must leave a deposit of $500.00.   On return of the incubator they will be charged $350.00 for use of this service.

Under no circumstances is this portable incubator to be used for Hepatitis Positive patients.

RE:  Dry Shipper
Patients other than IVFC or LP patients that are using our dry shipper will also have to leave a deposit of $500. On return of this shipper they will be charged $150.00 for use of this service.
