3. TIME OFF WORK
Those who require a medical letter for their employer or insurance forms completed, please notify the office staff. Allow 3 to 4 business days for the completion of such forms/letters.

4. MISCELLANEOUES CHARGES:

• Dry Shipper to Transport Sperm/Embryos (Maximum 2 days), $ 500.00.
  • An additional fee of $175/day in excess of 2 days will be charged.
  • Under NO CIRCUMSTANCES is the dry shipper used for Hepatitis Positive patients.
• Courier Fees – if applicable, current rate + admin charge, $25.00
• Copies of Medical Records, $50.00
• Insurance letters, $25.00
• Long Distance Charges & Faxes, variable rate, $25.00
• Return Cheque NSF, $35.00

5. UROLOGY SERVICES
- Epididymal Sperm Aspirations or Testicular Biopsies are billed by the urologist directly and payable to the urologist prior to the onset of procedure.
- Electroejaculation procedure payable to IVF Canada prior to onset of procedure,
$ 1000.00

6. Non-Funded patients who do not succeed in achieving a “Clinical Pregnancy” (i.e. confirmed by ultrasound five weeks post transfer) by completion of their second trial may qualify for a reduction in fees on their second and third trial (Please refer to our Financial Incentive Plan).

7.  Replacement of Treatment Package  
If a package was sent to you, and you have misplaced it a fee of $25.00 is applicable.

8. OFF-SITE BLOOD TESTS/ULTRASOUNDS
Please be advised that if you choose to exercise the option of having some of your blood tests and ultrasounds done off-site (ie: not at IVF Canada), you are responsible for paying the laboratory who performs the tests at the time the tests are performed.  If you have all of your blood tests and     ultrasounds done at IVF Canada, the costs of those tests are included in the fees you pay us; however, that is NOT the case when the testing is done outside of our facility.
