Determining Whether You're a "Funded" or "Non-Funded" Patient:
OHIP-Funded Patients:
If you are applying to undergo artificial inseminations (IUI), and you are an Ontario resident with an Ontario Health Insurance card, you are a funded patient. All of the medical services (i.e.: physician consultations/visits/blood tests/ultrasounds, etc.) are billable to OHIP. Services not covered under OHIP is the lab service of providing a sperm wash and any drugs used.
If you are applying to undergo in vitro fertilization (IVF), have bilaterally blocked fallopian tubes (both tubes are blocked), and the female partner has not reached her 40th birthday at the time of treatment, you are a funded patient. However, OHIP does not cover all of your treatment costs - you are required to pay for all IVF gamete laboratory services out-of-pocket; please refer to the attached pages of our fee schedule. (Both tubes must be completely blocked and this must be supported by medical records indicating tubal status).
Your initial physician consultation fee is billable to OHIP only if you provide a letter of referral from another physician - without the referral letter you are required to pay the full consultation fee yourself - currently $ 225.00.
Non-Funded IVF Patients:
If you do not have bilaterally blocked fallopian tubes, none of your costs are covered under OHIP; therefore, you are required to pay all costs out-of-pocket.
None of the fees quoted on this "Fee Schedule" include drug costs.
Please feel free to contact our office staff should you require any clarification. There are two columns of fees; one for “funded” and one for “non-funded” patients. Services in addition to basic IVF fees are NOT covered under OHIP, even for patients with blocked tubes.

- If a treatment cycle is cancelled, a refund or credit will be provided for the balance of services not provided.
- Some costs MAY be covered by OHIP in those patients with blocked fallopian tubes who have not already had 3 IVF cycles previously OR in patients with diagnoses other than blocked tubes who are either “grandfathered” or have achieved a pregnancy through IVF prior to April 1st, 1994.
NOTE: Some patients may wish to bring their own FSH supply, in which case any unused ampoules will be returned to the patient (we will NOT purchase extra unused ampoules). If you require more ampoules than you bring, you may purchase them from IVF Canada at the costs indicated.
IVF Canada will not reimburse or credit you for any tests done at outside laboratories.
